Emergency Evacuation Program (EEP)

Dependents of military personnel, non-emergency essential Department of Defense civilians, and contractors and their dependents may be subject to evacuation for their safety and security from the Japan area of responsibility when an evacuation is authorized by the U.S. Department of State.  To prepare eligible personnel and families for the possibility of an evacuation, sponsors are required to maintain a USFJ Emergency Evacuation Program (EEP) checklist and packet. 

The USFJ EEP packet is mandatory for all newcomers to CFAY who are eligible for evacuation.  To download USFJ checklists, fillable forms, and instructions on how to complete the forms, select from the links below. Once you have completed your USFJ EEP packet, notify your Command EEP Coordinator and provide them with a copy of your USFJ Form 178-R. Personnel having problems accessing the EEP Packet, right click on the above button and choose "Save link as..." to download the packet to a personal computer. Personnel having troubles downloading the entire package may choose the links of the EEP packet broken up into five sections. Personnel may need to right click, save the document to their personal computer and then open from the downloaded location.

The CFA Yokosuka Fleet and Family Support Center occasionally holds classes to assist families to properly fill out the documents and have the correct supporting documents readily available.
