Pass / Vehicle Registration / DBIDS Registration Office

 Visit the Pass/Vehicle/DBIDS Registration Office for all of your base pass and vehicle registration needs. The office is located on the first floor of the Security building near the Main Base entrance.

 Contractor’s Access (Official Business).  Each sponsoring command POC is responsible for submitting an appropriate pass request to Pass Office No Later Than (NLT) seven (7) working days prior to the first access date.  Reference: CFASINST 5500.1F (Access Control Instruction). 

 House Guest Pass.  An Escort Not Required House Guest Pass can be issued to SOFA sponsor’s or spouse's immediate family member (parents, grandparents, grandchildren, siblings and sibling's children under 21 years old) if the duration of stay is for "more than 72 hours but not exceed 90 days" in accordance with CFASINST 5500.1F.  The requirements are as follows:

1. Fill out the Request for Guest Visitation Form on both front and back, print out with two sided pages and obtain the appropriate Government Housing Manager's (Base Housing Director, NGIS Manager or Navy Lodge Manager) approval/signature.  The Request for Guest Visitation Form is available at Pass Office and the hyper link below. 

2. After the request is endorsed, submit it with a copy of proper identification (passport, Japanese Resident Card, etc.) to CFAS Security Pass Office for appropriate pass issuance NLT three (3) working days prior to the first access date. 

3. An Escort Required House Guest Pass can be issued to guest(s) who are other than immediate family members including foreign country national.  In addition to the requirement above (1 & 2), the guest's SECNAV 5512/1, PHS (Personal History Statement, USFJ Form 196aEJ/196bEJ) and a copy of proper identification, shall be submitted to the Pass office at least seven (7) working days prior to the first access date for an Escort Not Required House Guest Pass. The pass will be issued after the requirement is completed. SECNAV 5512/1 and PHS form are available at Pass Office and the hyper link below.

*United States Forces Japan Designated Third Country National TCN) Access of TCN Guest/Contractor whose nationality is listed on USFJ31-204 requires Commanding Officer’s prior approval.  The sponsor must contact Security Admin office (Tel: 0956-50-3620/81-956-50-3620) and complete the paperwork NLT 30 days prior to guest’s arrival.  The Access Request Form for Third Country National is available at Security Admin Office and the hyper link below. 

 Vehicle Registration You will need the following documents when:

 Registering a vehicle:

1.  Proof of SOFA status
2.  USFJ Form 4EJ POV Permit
3.  Japanese Title
4.  Japanese Compulsory Insurance (JCI)
5.  Liability Insurance
6.  Receipt of current Road Tax Payment
7.  Proof of approved designated parking space or Japanese Parking Certificate

 Registering a motorcycle:

 1.  Proof of SOFA status
2.  USFJ Form 4EJ Motorcycle Operator’s Permit                                                        
3.  Japanese Title
4.  Japanese Compulsory Insurance (JCI)
5.  Liability Insurance
6.  Receipt of current Road Tax Payment – Appendix 4
