CFAY COMMAND ADMIN DEPENDENT ENTRY APPROVAL OFFICE

CFAY Command Admin processes Dependent Entry Approval requests for all Navy personnel with families who have PCS orders to a command located at Fleet Activities Yokosuka, Yokohama, Tokyo, Yokota Air Base, and NAF Misawa.
 
Method of Submission:

  • Email to CFAY-DEA@us.navy.mil via MBR’s Personnel Office.  Requests must contain all Key Supporting Documents (KSD). Ensure sponsor's name is in the subject of the email

 
Required Key Supporting Documents:
  • DEA request message via message traffic to:  COMFLEACT YOKOSUKA JA

  • Member’s Permanent Change of Station/Assignment Orders with Modifications

  • Updated NAVPERS 1070/602 (Dependency Application)

  • Complete NAVPERS 1300/16 (Report of Suitability for Overseas Assignment)

  • Scan of Information page of No-Fee Passports for Dependents or proof of submission for a No-Fee Passport (DD Form 1056 signed by passport office) 


Additional Information:
  • Per MPM, request must be submitted at least four months prior to arrival

  • NAVPERS 1300/16 for the member and dependent(s) must be completed, signed and endorsed by member’s CO/OIC.  All pages are required (1-4)

  • DEA is processed in order of submission of complete DEA Packages
